from Aurelius Melas, son of Herakleides, grandson of Ammonius, also called(?) Diogenes, whose mother is Thaesis, from the illustrious city of Oxyrhynchus.
Having presented myself and having been examined at the quarter of Myrobalanos, and having been enrolled in the lists provided for the sacred grain allowance by the phylarch, according to the individual registers, but at the time of the examination I was unable to provide proofs of my lineage.
Now, necessarily providing the token of the examination and the proofs of my lineage, I submit these documents and request that I too may partake of the grain allowance, having been examined in the 15th year according to the decision of the most excellent council, equally with those similar to myself.
