English Translation

After the consulship of our lord Arcadius for the second time and of Flavius Rufinus, the most illustrious, in the month of Pharmouthi.
To Flavius Aetius, accountant (logistes) of Oxyrhynchus, from Aurelius Eulogius, public physician from the same city.
I was sent by your care from the documents delivered to you by Flavius Septimius Taulus, riparius (river official) of Oxyrhynchus, through his brother Phileios, in order to examine A[ ̣  ̣  ̣ ]phon, the peace-officer (eirenarch) of the village of Teeos, and to report in writing concerning his condition.
Therefore, I examined him and found his body dead, ̣  ̣  ̣ ri ̣  ̣  ̣ , without wounds, without marks, having succumbed to an acute illness; these things I hereby report.
I, Aurelius Eulogius, have submitted this report.
